I’ve tried to get 12V DC from output pins, but measured voltage
was 3,8AC.
Do I need to put some elements on board, or it is some mistake?

Outputs are relay contacts, so there isn’t any voltage coming out of them.
You should wire them as if they were switches.
I didn’t mean relay outputs, than output marked as 12+ (near input pins),
(in diagram, marked also as Vout).

Make sure that R20 (4E7) is mounted.
Current is limited to 200mA max.
R20 is mounted…
