K8055 Lockup

I have a K8055 connected to a relay board. The relays are 12V powered and the clamp is connected as usual. The relays switch 240V mains (400W load) and 12V DC (1.5A) to their respective loads. I can run the system for days whilst constantly switch the relays on and off - but when the load is connected occasionally (~1 in 50 times) the K8055 locks up and LD8 comes on constantly. The only way to recover is to unplug the USB cable for a few seconds and reconnect. As this application is intended to run unattended for long periods it causes a big problem. I have followed all normal good practice concerning grounding and decoupling so I have the following questions:- :question:

  1. What does the constant ON of LD8 indicate may have occurred
  2. Has this been noticed before
  3. Any remedies
